The most scary event happened after the Cold War in 1995 when Norwegian science team launched a rocket to study the aurora borealis. The team had warned several countries of this test, including Russia but for some reason this information was not passed to radar technicians. Tthe Russians were convinced that a nuclear attack was coming and they actually brought the Russian nuclear briefcase to Yeltsin in order for him to retaliate. Luckily, Yeltsin didn't buy was an attack and he never activated Russian warheads.
