Some of it would have to do with protectionists no longer being skilled professionals in the era of just hitting the 'play' button on a digital projector. You could easily see how someone just loaded the wrong hard drive in theater 3, hit 'play', and walked out of the booth. Likewise, I suppose you could argue that someone like that could just as easily be an unsupervised dick who did it on purpose... and yet another explanation could be that this sort of thing has always happened and there's just so much more reporting now that we hear about it every time now. As usual the truth most likely lies somewhere in the middle of all that.

Last year I read about an incident in which a trailer for Hereditary was accidentally rolled before the feature presentation of Peter Rabbit. This story reminds me of that.
