IMO probably Duncan Jones (Moon). Source Code was critically and commercially successful It wasn’t until Warcraft came out he started losing his touch. Warcraft wasn’t actually that bad either.

Herk Harvey. I knew da only movie he ever directed was Carnival of Souls (1962) but looking at his wiki page i was surprised to see he directed da short industrial machinery/construction workplace safety film dat pops up regularly on Turner Classic. Shake Hands with Danger.
I seen it a couple times and get a kick out of it. "I used to laugh at safety, now they call me Three-finger Joe".
Jared Hess? Was Nacho Libre a hit? His only movie i like is Napoleon Dynamite. Tried to watch Gentleman Broncos once on tv and only made to 25 minutes. So i didn't watch all of it but i consider it and G.I.Joe: Rise of Cobra da two worst movies i ever saw.

Billy Bob Thornton was considered practically the next Orson Welles after directing and starring in Sling Blade; he's only directed a few more films since, and they haven't received a lot of attention.

This one’s crazy because he showed insane talent and directed a pretty much perfect movie. Some of the best direction ever. Then he just dipped for good lol.

It was a box office and critical failure when it was released and Laughton so disillusioned, never directed again. It's reputation only began to grow years later.

Mark Pavia - While it was pretty much a commercial disaster since denied a theatrical release by the production company I always thought that The Night Flier (1997) was an excellent Stephen King adaptation, perhaps the best low-budget SK adaptation I can think of. The director was heart-broken after the movie went straight to video and hasn’t made another movie for 20 years.
Phil Joanou director of the great State Of Grace (1990) has been struggling ever since.
Michael Lehman who directed the cult favourite Heathers, has since mostly been working for TV aside from helming the huge bomb Hudson Hawk and some forgettable films.
While better known as a screenwriter for among other things The Harry Potter movies, Steve Kloves’ directorial debut The Fabulous Baker Boys (1989) was a solid critical and commercial hit. While I haven’t seen the follow-up Flesh And Bone (1993) it didn’t seem to be as well-received and put an end to further directorial efforts.

Joel Anderson (Lake Mungo, 2008)
Lori Silverbush (On the Outs, 2004)
Peter Mullan (The Magdalene Sisters, 2002) -made 2 more films, never even heard of them
Jean-Claude Lauzon (Léolo, 1992)
Michael Fengler (Why Does Herr R. Run Amok, 1970) -credited as co-director of Fassbinder, but actually this film is solely Fengler's effort,Fassbinder was nearly never involved in the making, his name only left in the credits for PR services. Fengler has 2 more feature films in his filmography, but this one is the only memorable one.
