For me it was in the Dirty Harry sequel Sudden Impact. The scene where Harry is doing target practice; when his partner Horace gets out of his car, you can clearly see the reflection of the cameraman in the car door window.
Anybody else?

The Terminator
During the factory chase at the end, the Terminator bot climbs some stairs and advances on Kyle. He then bashes it on the head a few times with a big metal pole. Well, on one of those bashes you see a big spring break and go wobbling around on the side of the Terminator's head! And then in the next shot, its perfectly fine. I remember thinking that they must have jerked the model's head a little bit too hard and broke it.

Con Air.
There's a scene where they open up a steel box full of pump-action shotguns and toss them to other convicts. When the other convicts catch them they're now M16s. I could be wrong but I don't recall any of the criminals using shotguns anywhere in the film, just M16s. Which is odd considering just how many shotguns there were in that steel box.

Reflection of the camera in the rear license plate. It happens in the opening sequence of Back to the Future Part II (1989).

BTTF II 1955 Biff's rearview mirror comes & goes. It's a poor goof because the mirror is key to Biff seeing Marty hanging off the tail with his hoverboard.
*Not a goof: the stairwell in that movie, the alternate ones that Marty tries to evade Biff & his cronies. I know it's real yet it seems a bit like an illusion - why are there alternate flights of stairs in the same place?

Man, good question, I'd have to go with the Stormtrooper banging his head off the archway in STAR WARS. I remember thinking, "Did that just happen?". But, since it was 1977, I had to wait a few years until I got the film on Beta to confirm it.

i think maybe the luke skywalker jump when he turns of the lightsaber in a new hope. he moves an inch to the left. but not until highschool so i'm sure their is earlier ones i've noticed. i like to see stunt doubles. beverly hills cop has some good one's i noticed back then too.
the pee wee's big adventure bike chain scene must be a big one for a lot of people. i always kind of figured it was on purpose the way it was so blatantly portrayed.

In Lethal Weapon, the scene where Mel Gibson handcuffs the guy on the roof and they jump. You can see the handcuffs break when they first jump. Why they showed it in slow motion to make it easier to see, I'll never know. Handcuffs happen around 2:14, you can see them break at 2:45:

"Superman II": General Zod and Ursa are able to talk and hear on the moon with no atmosphere. Even if they do have superpowers, they would at least need air to hear.
