A Serbian Film and The August Underground Trilogy. I haven’t even seen A Serbian Film. I read what was in it and have no interest in checking it out. Basically my jaw dropped to the floor and stayed there. It really pushes the bound of just about everything.

There aren't many. I'll watch The Exorcist and House of a Thousand Corpses all day. But one movie I am in no hurry to ever watch again is Antichrist.

I've NEVER watched Wolf Creek (2005) again because I COULDN'T STAND the female victims in it. STUPID BEYOND BELIEF... For me same but reverse reason: Over sadism. (and partly the reasons you gave)..thjis actually happened a decade or so ago in Australia.

I have never wanted to see Gorillas in the Mist or a later film-Instinct I think, because it is too depressing. I watched Babe and regret it for the same reason.
I wont see Salo because I know I would be too grossed out.
Documentary-wise, Animals In War is the most disturbing tv documentary I have seen.

United 93 was for its time. I bought it on dvd, watched it a good 3-5x over those years. I really don't need that when 9/11 can feel like yesterday morning if I opt.

That is a hard question to answer. I never get grossed out/disturbed by a movie enough to be discouraged to ever see it again, but one that has stayed with me is The Butterfly Effect (2004). I saw that years ago and haven't even sat through the whole thing, but in all honesty I cannot quite revisit it due to some of the insane things that happen. Particularly the sudden death of a mother and her baby. Just traumatizing in every way.

Yes (somewhat), but rather extremely very-loosely. Mick Taylor IS actually based on three (NOT one) Australian serial-murderers.

I haven't watched (and have no interest in watching) A Serbian Film or The Human Centipede or most of the other gory movies mentioned in this thread.
My answer is Lars von Trier's Breaking the Waves. I was blown away by how intense it was and by how fantastic Emily Watson's performance was, but I remember thinking to myself "There's no fucking way I am ever watching that again!"
