No all out Nuclear Holocaust is much less likely than it was in the 60's or 80's, the world is so interconnected right now specially economy wise (US-China for example) that a real Nuclear exchange between super powers is highly unlikely. even Russia has just too much too lose in a War with the US, we are no longer talking about an ideological conflict (Communism vs Capitalism) Russia wants to expand its influence and not endanger its own existence.
even smaller Nuclear powers like North Korea, Pakistan and India have nothing to gain from striking first against an enemy.
That being said a small scale nuclear conflict its still a possibility, lets say terrorists detonate a nuke in the US or Israel for example I can easily see either country striking back at any Islamic country they think/claim supported the attack with low yield nuclear weapons.
