My top 4:
Twelve Monkeys. Just about perfect finale to the criminally under-watched sci-fi serious. There were a ton of strands to untangle in the wibbly-wobbly-timey-wimey show and the makers did so expertly while also giving the audience a genuinely emotionally satisfying ending.
Ashes to Ashes. What a way to go out. It actually made me well up and get a lump in my throat on more than one occasion. The gruff luddite that is Gene Hunt is shown to be the hero we all knew he was . Great stuff.
Six Feet Under. This was just about note perfect in its ending but they had to (almost) ruin it with Keith's arc. His future cheapo sci-fi looking security get up was so hilarious it took me right out of the, otherwise brilliant, closing montage.
The Sopranos. I know this is hated by many but for me it worked really well. Perhaps it was because I only watched the show a few years after it had finished and was (kind of) aware of the closing scene. I thought it had a real emotional punch and the finality (and futility) of it all really hammered home the effect the extremely selfish lifestyle lived by Tony Soprano had had on those around him. Plus I never listened to Don't Stop Believing in the same way from then on.
