Smoking in recent films

I watch a lot of old movies and smoking shows up a lot but I wonder how much of it is subsidized. They say the main reason to do it is to give actors something to do with their hands but I cannot buy that since they can't show it in ancient historical films.
It's like someone having a glass of milk. A million drink options but chances are it is a glass of milk they are reaching for.
