Is that really a flaw? I don't think the film specifies that Ivy'41 is supposed to be an Englishwoman. From what I gathered from VIDEO WATCHDOG, it appears the remake's change of Ivy's last name from Pearson to Petersen was to emphasize her Swedish origins so Bergman's accent can fit (London was full of immigrants).
The switcheroo of actresses happened in '31 as well. Miriam Hopkins was to play Muriel the fiancés while Rose Hobart was to play Ivy the prostitute, but it was switched because Mamoulian or Hopkins found Ivy a better fit to the actress. Also despite what Ingrid has said, MGM records say that the fiancée (this time named Beatrix) was to have been played by Laraine Day. Of course, both words may be true.
