I'm not sure I agree with the examples. Both Joker & Amon Goeth belong in B for me. Still those are the villains I prefer. Ones that are distinctly opposite of what we think of as good. That's not to say the good guy needs to be their opposite as long as we can understand that the hero is flawed or has issues. The OP considers the Joker mentally ill and Goeth brainwashed, thus they fall into group A. Good point.
I guess I was looking at it as one I would feel sympathy for.
I don't feel sympathy for Joker just because he's crazy or mentally ill.
Still those are my favorite villain types
