My #13 all time. It's not a very sexy, easily accessible western to embrace, yet once I came around to it I fell in love.
There's some who feel it a letdown that Eastwood turned down a starring role for it, including being one of the Dollars leads who would've been blown away in the opening scene. The film is much about the end of days for western genre cliches, the new world moving ever slowly westward. Its DVD commentary was one of the finer educations.
Ironically(?) Leone's first choice for Fistful of Dollars was Bronson. The role went to Eastwood instead. That flipped for Once Upon... when Bronson was hired on after Eastwood turned it down. While Leone's vision for his characters is his own, I think both actors fit the roles they actually landed, best.
