i think my insurance would cover the orthodist, but I don't know about the insert. We don't have DME thanks These are what I use in my sneakers as well and I get them from Amazon or from my physical therapist's office. They trim to fit. These "over the counter" inserts are not all created equal. Most of them squish down and flatten and don't provide substantial arch support or have an adequate heel cup.  This exact brand only. Superfeet. The green ones, and full-length. They're $49.99 and worth every penny. Custom-made orthotics can run around $800 or even more if you're paying cash, but then again, they're very specific for each person, and made to order for each foot to correct any imbalances. But ^these^ are terrific alternatives. Inexpensive and they last. Thanks autumn I'll look for them!
