People are like, well i'll just order it without lettuce. They don't realize how much harder that makes their job. Some people that is.

Also depends on the type of restaurant. If I'm at a diner or something casual I have no problem saying hold the lettuce, to use your example. When fine dining, I get the chef wants the dish to go out as is.
