Their soul left their body, or their astral form left their body and they were able to see and experience the operating theatre as if they were in it.

I had had multiple surgeries (for cancer) and my anesthesiologist was my best friend's husband. He always did my surgeries.
After my experience with cancer, I had another surgery, on my foot. My anesthesiologist explained that I would be put under general anesthesia so I wouldn't experience the pain of the nerve block, and then the general anesthesia would be reversed. I would be conscious, but not remember anything that happened in the OR, and would feel no pain.
The next time I saw him, he inquired about my experience. I told him what the surgeon did and what tools he used, how I was allowed in a semi-sitting position so I could watch the surgery, I repeated jokes the two of them made about me not remembering any of it later. To say the least, he was stunned. He had to rethink what he told people about anesthesia. And I think he watched what he said in the OR after that.
