It seems to me that D.I.D. is a possibility as it takes many forms. The case of Kim Noble is particularly striking...

I haven't any immediate solutions to the puzzle, but a realistic dream sequence triggered by a mental disorder (D.I.D.) would I think be worth looking at, and may fit the story. I don't have the psychiatric nous for such a task.
I'll keep thinking about it, but I'd reckon that's the best I could come up.
