He wasn't that crazy really, what's been told about him has mostly been propaganda after his death.
The full accounts of his life were written by historians who lived after he died, including Suetonius, Tacitus and Cassius Dio. They were obviously biased against him. Plus, a lot of those accounts in turn contradict each other.
If you're going with Roman emperors, Caligula would have been a better choice as he was paranoid as hell (given the circumstances in which he grew up, I can't blame him). Although he too has been demonized by his enemies after his death.

Oh right, Andrew Kehoe. Double points for lunacy and villainy. He was responsible for the worst mass murder at a school setting in American history.
Because he lost reelection for a county position,for a clerk no less, among other things...he detonated an explosion at the elementary school, killing 45 people and injuring 58 more; 38 children died. Kehoe wound up killing himself after transporting more explosives to the disaster area once rescuers arrived, causing more damage. It's a wonder that he's not more well known. What an asshole.
