Some married couples BOTH use both of their last names with a hyphen connecting them.

Depends. If she wants to that is fine. If she doesn't want to that is fine too. I don't care if my future wife wants to keep hers. My last name is long and hard to pronounce; and my paternal grandfather just decided to start living with a new last name when he came to the US less than 100 years ago. This name has been a pain in the ass all my life.

its a "medieval" custom, I don't see why anyone Should do it, unless they really want to for themselves, maybe they like the new name better etc. Its a clear no from me. Don't see why I should give up such a big part of my identity. not saying i judge those who do it, on contrary, live and let live. so let US who DONT want to do it live also, without forcing judgment on why we chose not to do it.

When I got married I simply (well, I thought it would be simple!) added my husband's surname to mine, with a hyphen (-) to separate them. He also began using my maiden name with his surname on some official documents.
I don't recall my father ever commenting on this somewhat unconventional practice, but I got the impression that he was pleased.

Doesn't effect me; I don't care.
Should my next wife want to keep her last name, I would have no feelings on the matter. I honestly don't see why we still do it in the first place. Seems like one of those outmoded rituals that we've yet to kick to the curb.

As a lesbian I generally don't think about it 'cause even if I got divorced from my Wifey I would never go back to dating guys again but I think it is up to the woman if she wants to take her husband's last name or not. My Sister took my Brother-In-Law's last name when she got married to him but she wanted to get rid of our parents' last name and my Wifey and I took my Aunt and Uncle's last name so we could both get rid of our parents' last names too.
