Considering some of the governments and regimes that currently possess nuclear weapons and some of the governments and regimes that are currently trying to gain possession of them, I'd have to say yes.

But yeah, the 80s were a rehash of those fears.

I don't feel so but I am not an expert in the field. I believe there is increased pressure from various groups in favour of not using nuclear weapons and there may be greater internal control measures to safeguard against the unexpected attacks from within.

No because state powers are now investing hugely into non nuclear WMD. They don't want to win a zero sum war.
There'll be swarms of drones attacking cities with pinpoint accuracy killing millions, but at least we'll not kill all the butterflies.
To that point though, I think states like Israel & Iran will have to do with nukes, which they'll use as martyr weapons. Not a global catastrophe though - Iran, North Korea, etc will never ever have arsenal enough to erase continents from existence.

Less of a possibility now I think.
What you have a bigger possibility of now is either a terrorist setting off a "dirty bomb," which would be a mess to deal with, or some idiot, either intentionally or unintentionally, unleashing some sort of nasty biological agent that causes widespread problems.
War in general now seems to be done in the manner of a series of terrorist actions.

I worry about hubris that the fail-safes out there will leave some weapon somewhere open to some outside force with malicious intent gaining some sort of network access.

ALL OUT Nuclear war is probably less likely nowadays. Not impossible of course. As long as they exist it can still happen.
I'm more concerned with limited nuclear strikes, such as unaccounted for old nukes, unaccounted for new nukes, and terrorist use of nukes and dirty bombs. I think those are more likely. And its very scary.
The horrible thing is that if it happens its going to be in a large population center like a city. Terrifying.

No all out Nuclear Holocaust is much less likely than it was in the 60's or 80's, the world is so interconnected right now specially economy wise (US-China for example) that a real Nuclear exchange between super powers is highly unlikely. even Russia has just too much too lose in a War with the US, we are no longer talking about an ideological conflict (Communism vs Capitalism) Russia wants to expand its influence and not endanger its own existence.
even smaller Nuclear powers like North Korea, Pakistan and India have nothing to gain from striking first against an enemy.
That being said a small scale nuclear conflict its still a possibility, lets say terrorists detonate a nuke in the US or Israel for example I can easily see either country striking back at any Islamic country they think/claim supported the attack with low yield nuclear weapons.
