Among Dermot Mulroney, Dylan McDermott and Dylan Walsh, who all came upon the scene pretty much simultaneously, I'm still not sure which is which. I can always tell the three Reginalds apart, but when I see any one of them, I usually have to go through all three last names before settling on the right one:  Gardiner  Denny  Owen

David Keith and Keith David Then there was a Brian Keith and a was (is ) a Keith Carradine not to be confused with John Carradine But, your David Keith and Keith David example falls into that thing where the last name of one actor is the first name of another actor: For example, Gene Barry & Barry Sullivan and Robert Preston & Preston Foster

All three are easily pictured in tweeds, one hand clutching a pipe and the other thrust into a jacket pocket, but I couldn't imagine either Gardiner or Denny as a Scrooge; only Owen.

I know they look totally different, but somehowe played the same roles over the years. Fay Bainter and Spring Byington. When I see their faces I know who is who, but hearing their names I'm sometimes confused

I used to confuse two British character men, Nigel Patrick and Nigel Green, by name, not face or type. Totally different, but once upon a time when I saw the name of one I'd think of the other.
Was "dancing teacher" Arthur Murray, whose dance studios I remember well from my youth, ever confused with the similarly "profiled" (sic) Arthur Treahcer, ex-character actor, then Merv Griffin's TV co-host? Treacher later went into the fish and chips franchise business. Interestingly, there was a Treacher fish and chips restaurant not too far from the Arthur Murray dance studio in Boston. One was near the Public Gardens, in the Back Bay; the other in Park Square.
Not by name but by looks,mustache and their fondness for cigars, I associated comedians Groucho Marx and Ernie Kovacs.
Amazingly, I can recall being a child and having to think rea hard which Webb was which. Jack or Richard, I mean (and no, Clifton doesn't count; too different from the other two; nor the two Waynes, John and David).
June Haver and June Havoc, for sure. Only by name, though.
Then there were the two Armstrong guys, Robert and R.G. Also by name only.
Actor Harry Morgan (of MASH fame) once went by the name Henry, thus it could be easy to see his early screen name of Henry Morgan in the credits of a movie and think it was the radio comedian and TV game show regular.
Two regulars of old-time television who must have been confused, by name, by some viewers: King Donovan and King Calder.
I used to confuse a few actor-movie people with similar sounding names, and worked in roughly the same period: Donald Woods, Donald Cook and Eddie Woods. Also, Eddie Quillan and Eddie Borden.

Mildred Natwick & Mildred Dunnock ---I always get those two mixed up in my mind, Natwick tends to be the funnier of the two. Then there is Spike Jonze and Spike Lee, both current directors, I often think of Spike Jonze as being black because Spike Lee is black.
Laurence Fishburne and Samuel L. Jackson have even made a comedy sketch about people being confused about which one is which.

Albert Brooks and James L Brooks. Not related but often involved in the same movies. Incidentally, Albert Brooks was born "Albert Einstein".
To my shame, I still confuse Ewan McGregor and Jude Law. I never remember which of them was in which movie. I think they are both good actors, and handsome, but kind of bland.
