Not counting supernatural/superhuman beings that age slower (sorry Wolverine). I'd be hardpressed to think of one that beats "Harold and Maude"

It's animated, but Atlantis The Lost Empire with Milo and Kida. Kida is, I haven't seen it in a while, but hundreds if not over a thousand years old. Somebody could top that I'm sure, but with what I'm not sure.

In Entrapment (1999), then 69 year old Sean Connery has the then 30 year old Catherine Zeta-Jones as his love interest.

There has to be at least one movie with a very elderly man dating a girl in her late teens or early twenties that would beat Harold and Maude. I can't think of an example off the top of my head.

Brooke Shields and Keith Carradine in Pretty Baby? I believe she was 12 and he was middle aged if I remember correctly.
Still not even close to Harold and Maude where there's a 52 year age difference between the actors.

In the movie I believe their characters were 18 and 79 so it would actually be 61

There's a bigger age difference between the characters. The actual age difference between the actors is 52 which is still huge.

Breezy with William Holden and Kay Lenz. I don't know the actual age difference but I don't think it's as much as Harold and Maude.

Ha, it would never be done today. Having a guy much older than her, would be considered sexist and or icky. Just like when people complained recently about the show friends being sexist. Part of that had to do with the fact that magnum p.i. was in a relationship with one of the main cast.
I could be wrong I don't know.

Lolita (1997)
Jeremy Irons was 48. Dominique Swain was 15 at the time of filming it.

Woody Allen kept getting older and older while his female costars stayed the same age

What about Back to the Future III? Doc Brown of 1985 falls in love with Clara of 1885.
