I would pick Olive Garden with Longhorn Steakhouse a close second.

Don't really have one. BJs has a good Ahi Salad with wasabi, and jambalaya, plus TVs with sports and brews.
Taco Bell is decent, but I only get a 7-layer burrito with no sour cream and added chicken, and not often.
In-N-Out for a protein-style burger with both raw and cooked onions and chili peppers.

I have been traveling to the USA on business several times a year for 20+ years. I usually go to small/medium-sized towns and stay in the suburbs, so even though as a matter of principle I would prefer locally-owned restaurants, in practice I end up going to chain restaurants more often than not. It's practical, it's familiar, and I know what to expect in terms of quality, choices and prices.
Anyway, back to your question, I guess I'd say Olive Garden. I used to go to Applebee's a lot too, but not so much anymore.

McDonald's, I only eat the big Mac's . I miss the small family owned places that used to be so common in the U.S.

Red Robin
Five Guys
Longhorn
...any of the big chain fast-food joints. Never cared for Chipotle or Outback though.

I'm sure it's hazardous to my health, but once/twice a year I enjoy some KFC. I only like the chicken. I don't care for their slaw, or mashed potatoes. I bring it home, and make a salad of sort, to go with it.

Out of all the restaurants mentioned my small town has KFC and McD's. Of the 2 I prefer KFC, we get the tenders. We also have 2 Subways, both pretty much ruined by their new owners, and Quizno's not just ruined but as far as I am concerned killed basically because the new owner doesn't follow the Quizno's recipes.

Portillo's was originally a Chicago based chain but has recently expanded to Texas among other states. Their fast food is second to none...they serve a cheesy beef on a croissant that is to die for and a slice of chocolate cake that is heavenly. It is all good at Portillo's.

When I was in college in the south, I used to love me some Waffle House. Hot damm, a double Texas patty melt plate with double hash browns scattered. Perfection!
Couldn't tell you the last time I had it, but I imagine if I did eat it today, the time between finishing and darting into the bathroom would be rather brief.
