Yes partially, but the Anakin remains mainly responsible for his choices and destroying those he wanted to protect. It's a moral tale.

no, Force Majeure - the war was diabolically orchestrated, the Jedi were put into a tailor-suited dilemma they could not escape. A no win scenario.
- Should they have been better leaders?
They are not leaders, they are an executive force. Them trying to forcefully removing Palpatine (which democratically only the senate is entitled to do by vote of no confidence) was treason and gave Plaptine the legal ground to wipe them out.

I always felt that they were partially responsible. Their whole code: no attachments, no families, etc was created purely out of fear. They were acting on fear and played right into the hands of the Dark Side because of it....

Yes. Anakin didn't want to join the dark side. It was the people who were supposed to care about him that pushed him to.

Yes! It's a main point of the prequels! The Jedi were literally depicted in an Ivory Tower overlooking Coruscant. Let me find a video where they had to use Yoda to explain this in Rebels partly because so many missed that the Jedi were falling into darkness themselves as their own arrogance made them blind to the Sith.
And this great George Lucas quote that so many believed was only talking about the Republic, but it was really talking about the Jedi as well:

No, Anakin was an adult and made his choice. Yoda didn't even want him trained to begin with.
