Somewhat unsettling scenes rather than entire films:

(1976) Woman with far-gone expression driving away with carload of her "friends": Tourist Trap (1979) Woman puking up her innards: City of the Living Dead (1980) Boy impaled on hat/coat rack: X-Ray (1982) Woman removing her eye with fork and knife: Splatter: Naked Blood (1996) Kevin Spacey asking, "You like muscles?": American Beauty (1999) Girl unable to stop running until collapsing with bone protruding from leg: The Hypnotist (1999) Mother happily cutting away at her hand in the kitchen: Suicide Club (2001)

A Clockwork Orange (1971) Forbidden World (1982) The Green Mile (1999) Wolf Creek (2005) A Clockwork Orange is my favorite Kubrick film. I liked The Green Mile. Wolf Creek I found rather boring. Havent seen Forbidden World. I feel the same way about A Clockwork Orange. Wolf Creek terrified me. Easily scariest horror movie of the 2000s IMO. So realistic. Forbidden World is a fun old-school horror flick for those who like a certain type cheesy B-movie. The plot is a rip of Alien, but it's much gorier, sillier and sleazier. There's gratuitous sex and nudity and oodles of in-your-face gore. I wouldn't call it a good film exactly, but it's definitely quite watchable (and gross).

The first ones that come to mind are
Schindler's List Dogtooth Incendies Irreversible Trainspotting Visitor Q
Possibly the most disturbing is Happiness. The scene where the dad is explaining to his son why he likes little boys makes me shudder.

All the over the top gross out movies don't disturb me as much as they irritate. Most of the time, I can tell how hard the director is straining to upset the audience. "A Serbian Film" was completely ridiculous. Oh, yeah, show a baby being raped! Let em try and top that! Dumb film.
My list would be the following.
"Day of Wrath" (hands down) "Last House on the Left" "Happiness" (the father-son conversation) "Dressed to Kill" (for the elevator murder - very upsetting to me) "Passion of Joan of Arc" (Carl Dreyer was compelled to show human suffering.) "The Wicker Man" (1973) "The Witchfinder's General" "Tenebre" - (this movie has the highest number of upsetting murders, with the arm lopping bit being the most gruesome) "Short Eyes" - disturbing prison movie "Jaws" (not the movie as a whole, but Chrissie's death scene is my number one most disturbing death scene ever) "The Devils" (1970) "Chinatown" - (for John Houston's very OFF reaction to seeing part of his daughter's face being blown off and the very disturbing way he tries to comfort the young girl. You can TELL what is going on in his mind.) "Inserts" (1976) - just for the whole underbelly side of Hollywood during the depression. The movie was unfairly panned. "Monster" "Boys Don't Cry"

Mother happily cutting away at her hand in the kitchen: Suicide Club (2001) That reminds me of the woman sticking her hand in the pot of boiling water in the Village of the Damned 1995 remake. Even worse, after her friend removes it, she sticks it back in again while screaming the whole time!

Salo is probably the most disturbing. Martyrs is probably second. I've seen the uncut Serbian Film. It's a wild ride for sure, but it didn't really affect me much. Melancholie der Engle is definitely top three. It has everything. Real shitting, pissing, puking, and a fucking cat gets its throat slashed and killed. A fucking cat. It is the hardest of the hardcore extreme cinema you can get. One of the actors in the film changed his name on the film out of fear of prosecution. Rumor has it he filmed real necrophilia scenes that were ultimately cut. It's actually a decent movie. Beautiful cinematography and score. I felt the same way about 'A Serbian film' it was a bit over the top so not as shocking. as for the animal cruelty in Melancholie der Engle that's were i draw the line, i watch anything but when real animals are killed and tortured i look away same with a scene in Men Behind the sun where a cat is eaten alive by thousands of Rats. that shit is just too messed up. A Serbian Film is over the top and just such a dodo bird of a film.

Maybe its because I have an absolute sick/tasteless sense of humor but Happiness was flat out hilarious to me. Phillip Seymour Hoffman prank phone calling that woman took me out. But yeah I can totally understand how that scene you described would be disturbing. Crazy film all around haha. Also I must ask, just how fucked up is Dogtooth? Its been on my watch list for quite some time now but I haven't mustered up the courage to watch it yet lol.
