The director/writer did serve in Vietnam so its realism is probably valid.

Fragging happened a lot. Term came from "fragmentation grenade". A popular method was to roll a grenade under an unpopular officers bunk. And it wasn't begun in Vietnam. A sergeant in the British Army was killed by his own men after the Battle of Blenheim, 1704. Confederate general Braxton Bragg, as beloved as a fart in a crowded elevator, survived several attempts on his life. There were over 900 fragging attempts in Vietnam from 1969 to 1972, resulting in 99 deaths. And those were only from grenades. Something like Taylor killing Barnes in the film wouldn't have been reported and happened a lot, I would imagine.
Why, the US armed forces in Vietnam were undisciplined.
Awesome film. The death of Sgt. Elias is one of my all time favorite scenes in a war film
