How did you interpret the last 20 minutes?
I have seen some people say they think he died of a drug overdose, some that take it all at face value etc.
Personally I think the "happy" part where everything seems to work out is a dream and that the very ending shows that he is just going to get worse and having him encounter somebody who changed their life around because of something good he did early in the movie is sort of bittersweet.

I haven't seen it in a long time. My take on it was the Herzog Bad Lieutenant was a more positive riff of the 1992 original. Abel Ferrara's original 1992 film was unforgivable and Wernor Herzog's version was more forgiving.
