Nothing in North America comes even close to some of the rivalries that you can find among supporters of some soccer teams in some European and South American countries. Lazio and Roma in Rome, for instance, or Boca Juniors and River Plate in Buenos Aires, or Galatasaray and Fenerbahce in Istanbul, or Rangers and Celtic in Glasgow (that one being exacerbated by a Protestant/Catholic split).

Boca v River Plate is completely nuts. Only Celtic v Rangers comes close in terms of craziness.
I think the best 3 way rivalry is in Athens - AEK, Olympiacos and Panathinaikos.

There has never been an encounter between those two teams that didn’t end in bloodshed.

In terms of actual quality, England and Australia in Ashes. Aus have dominated more in last 100 years but quality of matches bar 90s have been higher than extremely boring games like that of Inter and AC Milan. Or that boring draws you see in Portugal big 3 games.

The 'best' we've got are some intra state & provincial rivalries because they feel like family fights, & fights within a family can feel more boundless than neighborly ones: Giants-Dodgers Rangers-Islanders Flames-Oilers Penguins-Flyers Cubs-White Sox Canadiens-Nordiques was a beauty, easily found on youtube. Those can all be far more sincerely fiery than inter geographic ones like Toronto-Montreal or Boston-New York.

India v Pakistan in ODI cricket in particular has had its moments. That World Cup Quarter Final in India in 96 had riot written all over it had the home team lost. And to show how accurate that statement is: an even bigger crowd did riot when India lost the semi final to Sri Lanka.
Australia playing the West Indies in the 1970s was also quite heated. There was a game in Jamaica in 1977 when the crowd rioted late on day 4 with Australia about to win and the riot spilled out into the city and at least one poor fella was shot dead in the chaos.

Yeah, I think that in order to have a really heated rivalry you need two teams from the same city. While practically every major city in Europe and South America has more than one professional club playing in the same league, this is really rare in North America.
