For example, Debra Winger was originally cast in the lead role for Broadcast News but backed out right before filming after getting pregnant and was replaced by Holly Hunter.

Jean Claude Van Damme originally played the title character in "Predator" (there's even footage of it), but he was seen as too small compared to Schwarzenneger, Ventura, and Weathers to be threartening looking. Also he was a bit of an asshole. They quickly replaced him with the 7 foot actor Kevin Peter Hall (he also played the Predator in the sequel)

Viggo's so associated with that role now, it's weird to imagine someone else playing it. Yeah they had already started filming and Peter Jackson decided Townsend was too young to play Aragorn. That had to be a tough conversation. You fly halfway around the world to be a central character in a huge blockbuster franchise, just to get told to buzz off.

Debra Winger was also slated for the title role in Peggy Sue Got Married, but was unable to take the role due to an injury and was replaced by Kathleen Turner.
She was also cast in the role of Dottie Hinson in A League of Their Own, but backed out at the last minute because she did not want to work with Madonna. (Madonna, for her part, was not the nicest person on the set; unlike the other cast members who were friendly with the extras, entertaining them with jokes [Rosie O'Donnell] and puppet shows [Tom Hanks], she refused to be so chummy, and also made some ugly remarks about director Penny Marshall, complaining that "Laverne" from Laverne and Shirley was her director. Ironically, the movie was probably the best [or only good] one of her otherwise mediocre film career). She was replaced by Geena Davis, who seemed like a much better choice anyway, with her striking model looks.

Judy Garland for ANNIE GET YOUR GUN. She was casted for several pictures with Fred Astaire to repeat their EASTER PARADE success but Garland’s problems led to her being replaced in THE BRKLEYS OF BROADWAY (Ginger Rodgers), THE BELLE OF NEW YORK (Vera Ellen), and ROYAL WEDDING (Jane Powell).
Jan Francis’ anemia made his acting in ALIEN impossible, leading to John Hurt taking over.
Jean Harlow replaced Greta Nissen for HELL’s ANGELS when the silent was made into sound and the latter’s Norwegian accent (she played an englishwoman) stuck out.

Chaplin fired Virginia Cherrill as the Blind Girl in CITY LIGHTS and tried to replace her with Georgia Hale. But realized he couldn’t afford a full refilming after so much time and footage was shot so he rehired Cherril.
Buddy Ebsen as the Tin Woodman (suffered breathing and muscle reaction to the Aluminum dust makeup). Jack Haley.
Tyrone Power as Solomon in SOLOMON AND SHEBA (died from a heart attack during a sword fight scene with half of his footage filmed). Yul Brunner.
Peter Finch as Caesar and Stephen Boyd as Antony for CLEOPATRA. After the England shooting was abandoned (with little shot due to Liz’s chronic health problems), they also dropped out.
Sean Young as Vicki Vale in BATMAN (injured her leg riding a horse for a scene that was eventually cut from the filming). Kim Basinger.
Annette Bening as Catwoman (got pregnant). Michelle Pfieffer

Sergio Leone offered the role of Juan Miranda to Eli Wallach, but Wallach had already committed to another project. After Leone begged Wallach to play the part, he dropped out of the other project and told Leone he'd do his movie. However, the studio already had Rod Steiger signed. Leone offered no compensation to Wallach, and Wallach subsequently sued.

Jennifer Jason Leigh and Harvey Keitel shot scenes for Eyes Wide Shut, but due to scheduling conflicts, they were both replaced with Marie Richardson and Sydney Pollack.
