1. Didn’t Bruce Willis just tell Cole he was his shrink? I don’t know that he was actually assigned anything.
2. Yes, I believe so.

Maybe he was assigned to Cole’s case before he was killed.

I don't think he was assigned, I think he was drawn to Cole's energy or whatever. Cole says something about how the ghosts are attracted to him, ad they don't even realize they're ghosts. One of those lines that really sticks out on rewatch.
Cole definitely knows he's a ghost.
