The decent documentary mentions that Okinawa was the only battle in WWII that both commanders did not survive. US commander Simon Buckner was killed by a shell fragment and Japanese commander Mitsuru Ushijima committed suicide. In all wars, a major battle where both commanders died is a rarity. The only other one I can think of is Battle of Quebec, of The Plains of Abraham, in the French and Indian War. Both British commander James Wolfe and French commander Louis-Joseph de Montcalm wee killed. In the 1389 Battle of Kosovo, both leaders, Ottoman sultan Murad I and Serb ruler Lazar died. Lazar was probably killed in battle but Murad was murdered right after the battle. I cannot think of another big battle where both commanders died.

Ironic, two of the three are in the same war. And Ushijima is a bit of a stretch. He probably wouldn't have died in battle. I know of a zillion battles. Many who had one commanding officer die. But only these in all the battles of human history.
