It is certainly very brutal. I remember watching this at about 3am on Netflix taking a break from working on my final thesis for grad school. I popped it on just wanting to take a break before going to bed, but it was so alluring and disturbing at the same time, I couldn't take my eyes off it. Very effective film in this regard.
As for the worst of all time, "The Fly 2" has it for me. The guard who gets the acid spray, tears off his own face, and is still barely breathing when his coworkers find him is the kind of stuff that lives in my nightmares.

Not sure, but it has to be up there lol.
Some others would be the Driver bashing that goon's head in during the elevator scene (Drive)
Then imo the worst most brutal thing I've ever seen is in The Killer Inside Me. For those that have seen that film (not many people have), you'll know what I'm talking about. Horrifying!

Actually scratch that, Bone Tomahawk features the most brutal /horrifying death scene I've ever came across.

I was always partial to the liquid nitrogen kill in Jason X, myself.
