Worst was double sided discs and I could NEVER remember what way to put it in for widescreen, and sure enough, that screen would pop up on first try EVERY TIME.

The other one that really annoys me is nowadays when they say the film company doesn't represent the views expressed in the commentary. Has someone sued one in the past or something?
