At over 50 y.o., I've seen a lot, certainly not all of the extreme or avant-garde ones, but, Seconds (1966) by John Frankenheimer is at the top of my list. I like it a lot. Not sure if I would classify it as "horror," but it does have a creepy "Twilight Zone"-ish vibe.

Kind of a similar idea to Fight Club--the wimpy guy wanting to reinvent himself.
A similar film but with a different ending is THE MAN WHO HAUNTED HIMSELF starring Roger Moore. Highly recommended. Example of what filmmakers could do without Hollywood meddling.

I like it a lot. Not sure if I would classify it as "horror," but it does have a creepy "Twilight Zone"-ish vibe. That's what immediately popped into my head. There's an old Twilight Zone out there about a guy trying to find the fountain of youth and he gets younger over night. There's also a Tales From the Crypt out there how an old rich man has a procedure to pretty much make himself younger - pretty much gets a whole new body/face.....

I meant, having seen 50 + years of movies, I'd say the scariest, IMO, is Seconds. Of course, I would have said Jaws in the 70s.
