I remember the director's commentary for the DVD of The Exorcist being pretty bad. Instead of any kind of information about the story, the actors, his techniques, or just some trivia about the making of the movie, William Friedkin acts instead as a descriptive service (used for visually impaired people), merely mentioning what's happening on the screen ("and now here we see Ellen Burstyn walking into the church!" etc. Yeah, we see it.)

Most disappointing Masters of the Universe--Gary Goddard alone. So boring. Mostly talks about camera set up.
Good ones
The Howling-with Joe Dante, Dee Wallace (best part, when she is watching the nude werewolf scene while husband Christopher Stone is sitting next to her), Robert Picardo (he hardly says anything!).
They Live -- Carpenter and Roddy Piper.
The Thing - Carpenter and Russell (but Carpenter isn't as lively as he is with They Live)
Scars of Dracula--maybe my favorite audio commentary --Christopher Lee and Roy Ward Baker--so very informative.
You never know with commentaries--like Christopher Lee is really chatty in the Dracula one, but in Rasputin-the Mad Monk he spends all the time talking about Russia history and it is so boring.
Dr. Jekyll and Sister Hyde--Roy Ward Baker again, with Brian Clemens and Martine Beswick.
Asylum-again with Roy Ward Baker but not so good--the other guy is DOP or camera operator and it is just not so interesting.
The Blood on Satan's Claw --Writer, director, and star Lynda Hayden. One of the best.
Corruption-- in this case no one associated with the movie but the historians are pretty good for a movie that is not well known.
My favorites would be

Two really good ones stand out for me. First there was the commentary track for Death Wish II by this writer who is an expert on the Death Wish franchise. He was very thorough, and even gave info on all actors who had bit parts. The commentary on the Criterion edition of Straw Dogs by a Peckinpah historian was also thorough and he explained some hidden meanings behind some scenes.
As for a notably bad one, I found the commentary for Omen III: The Final Conflict by the director, Graham Baker, quite bad. He didn't seem particularly enthused about doing it, and as a result it was very dull. Too many long pauses.

This is Spinal Tap - The boys do the commentary in character. It's as hilarious as the movie.
Mr. Show w/ Bob and David. They are mostly doing audio comedy skits.
Newsradio. Very funny, although they sometimes go very off topic.

Fight Club - Fincher / Pitt / Norton are all very entertaining & informative . Also, Fincher cusses quite a bit and is pretty damn funny .
Orgasmo - Trey Parker & Matt Stone are funny as hell on it and were getting tipsy through out the commentary .
