There's a stereotype in mainstream films that exaggerate gay characters in some aspects, like being overly flirty, soft-spoken, or cheerful that makes it seem like every gay person behaves that way when it really isn't true. Actors should be comfortable playing their character irrespective of their true sexuality.

Iโm so stupid I naturally assume that any actor who has played a doctor is actually a doctor.

A good actor should be able to play any character. There might be certain limitation by age, height, color etc. pp*., but generally he or she she be able to pull it off. Patrick Stewart would hardly convince as high school student due to his age, Peter Dinklage would not fit as Goliath in a historic drama about King David, and a historic figure like Dr. Martin Luther King should be portrayed by a black actor. But being gay, a communist, deaf, blind, a racist, a drug addicted, a genius etc. pp. are "abstract" qualities.
