White Castle but I moved from the Chicago area in 2001 and haven't had a fresh White Castle since that time because they don't exist in Nevada, New Mexico or Alabama. The frozen ones don't taste the same.

Well, I refuse to call fast-food places "restaurants", so I will not put McDonald's or Subway up for consideration.
I travel to the USA for work several times a year, usually to medium-sized towns, so I often eat at chain restaurants. In the big cities like Chicago or Los Angeles or Boston or New York I would never go to Applebee's or Olive Garden or Outback, but in smaller towns or in suburban areas, chain restaurants provide a familiar and "safe" (as in "I can be reasonably sure that the food will be decent") option.
The one I have visited the most times is probably the Olive Garden, with Red Lobster in second place.

That would be Denny's, because I've used them as my "go to" stops when I have driven to Oregon to visit school friends. I have 2 routes ... I-5, and 101. It's a 600 "plus" mile drive, and I divide it into 2 days. There's a Denny's near both of the hotels, where I stay, so that can be dinner and/or breakfast. And, there are several where I can stop for a mid-day break and meal.

McDonalds, easily. Especially during the days when I used to get the happy meals.

Tough one. Maybe Taco Bell, but I never even saw one until my mid-20s.
