If you mean fictional doctors in movies and TV, my choice is easy. It is not a doctor, but THE Doctor, the genuine article, you might say. I talking, of course, about the title character of the long-running (its 37th season in 55 years begins in October) BBC science fiction show, Doctor Who.

Easy: Dr. Richard Kimble, on the popular TV series The Fugitive. David Janssen gives the best, most naturalistic and realistic portrayal of a doctor I've ever seen in a TV series or, off the top of my head, in a movie, either. I've known many doctors, have worked with them, my generation and much older, and I've never seen an actor nail a doctor character like Janssen did when playing Dr. Kimble. Also, the more he was called on to use his professional skills the greater his credibility, with me anyway.
(I'm deliberately using widely known mainstream movies and TV as my reference points. If I pondered the issue in greater depth I could probably summon up the name of a British or European actor in a (most likely) little known film seen by few Americans, but I'll pass. Actually,--okay--two come to mind: Liv Ullmann, as the extremely put upon psychiatrist in the excellent, deeply moving Ingmar Bergman Face To Face; and James Donald as the sole medical man in the prison camp in The Bridge On The River Kwai. Donald wasn't given much choice dialogue but his performance was outstanding.)

My favorite would be Dr. Frederic Steele as played by George Brent in DARK VICTORY (1939). The last three years of my life have had me deal with many doctors. I came close to having a doctor like him, but he wouldn't take my insurance.
