It would have to be Jets over Colts in Super Bowl 3, or Giants over Patriots in Super Bowl 42. I'll say 42.

Jets +18 over Colts
Giants were only 12 point dogs in SB42.

SB 42 - New York Giants defeat. New England Patriots, 17-14 Was that the year when the defeated team had won every single game in the season before losing the Super Bowl?

One that should be mentioned is Denver beating Green Bay.
NO ONE thought that the Broncos were gonna win that one. I'm pretty sure the Packers were a double digit favorite.

One that nobody has mentioned yet is the Pats over the Rams in SB XXXVI. Rams were favored by 14.

I actually saw that upset coming.
In the last several weeks of that season, The Rams always looked and played like they were tired as heck and they definitely went into the Playoffs feeling that way. That 1st Round BYE helped them get past their First Opponents but then again, they really looked tired in their NFC Championship Game against Philadelphia which they were still able to win and then instead of getting 2 weeks off for the Super Bowl, they only got 1 week off which wasn't enough time for the Rams to fully wake back up and so they went into the Super Bowl tired and worn out and we saw that.
Plus, when your team is feeling like that and your going up against a young Tom Brady, Bill Belichick, and the New England Patriots......you basically have no chance.
LOL, I actually still remember how towards the end of the Rams final game the very next season.....John Madden actually figured out what the problem with the Rams was which was that they were worn out.

Yeah, that as the one where Scott Norwood barely missed a very difficult FG.
