Peter O'Toole's replacement comes to mind first, against Quinn the Eskimo in SAVAGE INNOCENTS. That's obviously the most famous example, except perhaps Darth Vader.
I wonder if you include Perry Mason's visit to Japan as dubbed dialog? It's quite obvious that Burr was never in the original movie, but the movie is classic enough that his addition doesn't hurt it.
