"Wake in Fright" would be a good example. It could all be true - nothing supernatural, no CGI... "The Housemaid" (1960) is another example, but I didn't like it. "Rosemary's Baby" would NOT be a good example (I won't spoil why)... "Eyes Without a Face" and "The Face of Another" (even better) are very good, and have been classified under this category.
"Fist in Pocket" (Bellochio) might be one of my favorites, if not my favorite. Italy, 1965ish.
The original "M" might fall under this category, but I think many would consider it a thriller/mystery, but its great.

Does “realistic” just mean non-supernatural?
If so, here are a few:
Murder by the Clock (1930)
Freaks (1931)
The Old Dark House (1932)
Murders in the Zoo (1933)
Mad Love (1935)
The Seventh Victim (1943)
Psycho (1960)
Repulsion (1965)
Several others are on that cusp of supernatural-or-not?

Psycho and Texas Chainsaw Massacre were actually inspired by a real serial killer (Ed Gein)

Never saw "Chainsaw", even though me and my brother tried putting the VHS in years ago... It didn't work, and when I gave it a shot a few years ago, I couldn't get into it.... Oh well, maybe someone else might? Cheers!

Realism didn't really become a major component of horror until the 70s IMO. A few that would suit that description:
