Yes, I have. At least 20 years ago at Gurnee Mills in Illinois.
I remember a lot of Hollywood memorabilia on the walls and the food was decent.
I remember the police motorcycle (either intact or parts of it) Mel Gibson used in Lethal Weapon 3 hanging up in the air or put on a wall. I also remember a leather jacket Arnold Schwarzenegger used in Terminator 2 either being worn by an Arnold mannequin or hanging on a wall.
I've also been to the Planet Hollywood hotel/casino in Las Vegas.

Toronto, 2003'ish. Was alright, standard casual dining hamburger combo plate I guess. Maybe just a handful of A-list memorabilia, a bunch of Canadian celeb stuff helped.
I still have & use on occasion a tall souvenir pint glass. All these dishwasher years later, its label has barely vanished.
I went to the one in London with my then wife about 13 years ago. Saw a viewing of a movie and had burger and chips. When got home I was violently ill from all areas and camped in the John all night, missed a night's sleep and a day off work. My wife was fine.....she is a vegetarian.
I wrote to them in a pique of annoyance , to which they replied with an apology.......and guess what............2 complementary tickets to go again !!!
We didn't.
As an aside, I remember Mike Aspel said one of the final straw reasons he quit interviewing celebs on telly was because of his excruitiating awareness that Sly, Bruce and Arnie's only reason to come on the show was to plug their restaurant.

Was at the one in New York City in late 2007/early 2008. Food was alright. They had a bunch of stuff from the Die Hard movies including an empty water jug, a gold bar, and McClane's shirt from the second movie.

Went to the one in St. Louis, sometime back in the late 90s. Don't remember much about it other than it being over-priced, and some weird menu items (Demi Moore's Captain Crunch battered chicken strips). Some marginal memorabilia, nothing that really stood out to me. That was the only time I ever went.

A couple times, when I was young we went to one in Niagara Falls CA, and a few years ago we flew into NY late and PH in Times Square was the only restaurant close to our hotel without a ridiculous wait time. The food is nothing spectacular, just TGIFridays with a fancier presentation. As far as props my favorite thing they had was the automaton from Hugo and there was a Walking Dead zombie puppet leering over our table the whole time.
I went to the one at the Houston Galleria years ago. I don't remember that much about it, nothing really stood out to me at the time. The food was average which is what I think hurt it in the long run. Once you went and saw the memorabilia if the food didn't have you coming back you probably wouldn't.

The only time I ever ate at a PH was in Houston in 2009. My Lovely Wife and I were there to catch a Gilbert & Sullivan Society production in the theater district. I seem to remember it was a downtown location but now I can't find a trace of a Planet Hollywood anywhere in Houston.

Yes, thanks. I probably have mixed up Hard Rock with Planet Hollywood. Well then, I have never visited a Planet Hollywood, so there!
Having other people mention the Hard Rock cafe, I do remember going to the, also closed now, Harley Davidson Cafe in New York.
All the motorcycles inside, the atmosphere and the food was nice. The prices fit the the whole thing though.
