They’re both leads. It’s a film about Amin as seen through the eyes of his fictional doctor.
It’s hard to imagine how anyone could seriously consider McAvoy a supporting character in this film.

Watch the old documentary General Idi Amin Dada and this film back to back. It'll give you a good appreciation about Idi Amin. The filmmakers and Whitaker in particular did an excellent job conveying his personality.
Back to the topic though, I always figured they were both leads.
