Martyrs (2008) is a must see for extreme horror buffs. It's some hardcore shit. The director apologizes for making it on the DVD's introduction.
Salo, Forced Entry, and Melancholie der Engle are probably the three other really hardcore movies I've seen.
Honorary mention goes to Megan is Missing. That movie will ruin your week too.
A Serbian Film didn't do much for me. It's almost too silly.

A Serbian Film
An offensively disgusting movie. Unlike the poster above I find it extremely effective at being disturbing, regardless of how OTT the story is.
Once the movie starts showing infant rape I didn't watch anymore and I can't endorse this movie on any level. The movie is basically an endurance test.
The movie made me sick.

Salo, Forced Entry, and Melancholie der Engle are probably the three other really hardcore movies I've seen. Honorary mention goes to Megan is Missing. That movie will ruin your week too. A Serbian Film didn't do much for me. It's almost too silly. It's funny because I find A Serbian Film way more disturbing than Megan is Missing. I was easily able to finish that movie and it was only effectively disturbing during the last 15 minutes or so. The real time way the last act does things is tedious though. It didn't even ruin my day. The movie is a mostly ineffective and almost laughable low-budget faux documentary.

Of the movies mentioned none gave me that type of feeling, except maybe Paranormal Activity. I do find that movie scary, but that is because I find the concept of demons terrifying. I don't even believe demons exist, but the idea is very scary and the movie does what it does well. Megan is Missing doesn't imo. The movie is amusing trash at best, with a bit of torture porn thrown in. The Inkeepers I just find boring and didn't even creep me out. The Village is a decent movie, but not even close to one of the scariest movies I have seen. I find so few movies scary or overly disturbing and that is why A Serbian Film came to mind immediately. Cannibal Holocaust is absolute garbage, but that movie is one of the more disturbing horror movies I have seen. Hardly scary and quite boring, but it is disturbing. Movies like Cannibal Holocaust, Megan Is Missing and A Serbian Film are a separate category of their own. I call that category, pretentious disturbing porn. Movies that serve only one purpose...to be disturbing. I'm not sure what Megan Is Missing is even going for exactly. I think it is trying to pass itself off as a warning to teenage girls of the danger of talking to random people on the internet. I suppose it could work in that way, as tacky as the movie is. It is like an R-rated after school special.

Requiem is disturbing in a different way. It is more unrelenting in it's intensity and really makes you feel for the characters. It is a movie that is visceral and surreal. The difference between these and the other movies mentioned is that they are exceptionally well made, interesting, well-written and have good characterization. They pull you in emotionally from the start and never let go.

Unfortunately I lost interest in the story at about the half-way mark, but there are many things the movie does right. There is a good movie in there somewhere. I prefer the remake, because it is more self-aware.

Not really sure why. Maybe because it is one of the most creative and unique movies ever made. I have never seen a movie quite like Eraserhead or a movie that captures the feeling of a real nightmare as well as Eraserhead. Stanley Kubrick said he wished he had made it. That is some very high praise.
