The names of the cast will appear in every episode, just the names of the actors, no character names. And then, at the end, one (seemingly random) member of the main cast will appear along with the name of their character.
Examples…
Everybody Loves Raymond: Ray Ramano, Patricia Heston, Brad Garrish, Doris Robert, and Peter Boyle as Frank.
Seinfeld: Jerry Seinfeld, Julia Lewis Dreyfus, Michael Richards, and Jason Alexander as George.
What’s the deal?

everyone is credited at the end with at least TWO names except Gavin MacLeod as MURRAY

"with" & "and" credits are negotiated to recognize clout and/or prestige That's^ pretty much what I always figured was the reason for it. That and as an indication of how important the character they play is to the show. First time I remember seeing it was in Buffy the Vampire Slayer with Anthony Stewart Head who played Giles.  Once he left the show, Alyson Hannigan got his position in the opening credits and the 'And/As' that came with it.  While on the spin-off, Angel, it took until about the fourth season (I think) for Alexis Denisof to receive the 'And' credit (no 'As', though).

He was the youngest actor in the main cast, but he was the title character.
