The painting Madonna of the Streets by Roberto Ferruzzi (1854–1934), is mostly used as a painting of the virgin Mary and baby Jesus, but its not a painting of Mary and Jesus at all. Its a painting of Angelina Cian (age 11) and her younger brother, its now used by many as a religious picture, but it was not painted as a religious picture and has nothing to do with Mary, Jesus or religion at all. Its just a painting of a sister holding her brother. There are many copies of the painting, but the original painting that Roberto Ferruzzi from 1897 is thought to be lost.

Well, OK, but the fact that Ferruzzi titled his painting Madonnina (little Madonna) indicates that he saw the girl and her brother as representations, in some way, of Madonna and Child.

What is interesting with Madonna is that it was first attested in 1552 and it just meant my lady, it was not until 1844 it was used in a way to describe the virgin Mary. The meaning of the name before 1844 was just my lady or Italian woman. Well i find it interesting.

I see what has happened, i looked at Hungarian From Italian madonna, from Old Italian ma (“my”) + donna (“lady”). It was first attested in 1552 and its meaning was primarily (Italian) woman. Its use in the sense of the Virgin Mary was attested much later, in 1844 en.wiktionary.org/wiki/Madonna

I always thought that it was an image of a young mother with her child ..the clothing indicating that they were not rich or "gentry". To me she looks much older than 11.

I agree. she looks older then 11. I also thought it was a mother and a child, but its a sister and brother. "Madonna of the Streets" as a title makes sense as in a poorer family, an older sister might ( quite likely )be having to be "mother" to a younger sibling while their mother is out working.

It does look like a standard religious painting of Mary and Jesus used in home worship, it’s even kitschy in the way of pious commercial art. That the artist references the girl as a Madonna, when she not a mother but a sister, perhaps means this is the reaction he expects. In pop Catholic culture, the Virgin Mary is thought to have been around 12 or 13 years old since that was a normal age to marry daughters off. Of course, the Bible never says how old she is.

I'd tend to agree that I don't think the artist was unaware of or unwitting in his obvious use of religious connotation here. Apart from that, it's the usual Victorian kitsch that artistic ultra-conservatives (such as Evelyn Waugh) tended to laud as the ne plus ultra of painterly achievement.
