Thing is clearly BS! Janet Leigh is given the “and” credit which is for a notable actor that has a SUPPORTING role. Not the lead. The “people were shocked because people thought Janet Leigh is the lead” thing is one of those “fun facts” people like to say but it is clearly not true

Back then, I don't think people were as in tune with what particular credits meant and how they were worded. So I think people just saw her name in the credits, paid it no mind, followed her for the first part of the movie and were surprised when she was killed off.
I think the bigger question is why it came as so much of a shock when the movie was based on a novel that a lot of people had presumably read at the time.

Yeah, but how many people do you think read the novel? Even if it was 1 in 5 (which I doubt) there would still be plenty of people who didn't. Not to mention the movie was released only a year after the novel.

Her image is much larger than that of anyone else. As for why people didn't know the twist because of the source material, according to IMDb trivia:

In addition to what alpha said, Hitchcock made it so you couldn't be admitted to the movie if you were late so that everyone watching would be introduced to Janet/Marion as the lead. He also made a trailer for the movie where he swapped Janet with Vera Miles, so keeping her death a surprise was clearly important to him.
We're also talking about something that had never been done before. It's been imitiated since with your Ned Starks and Drew Barrymores, but in 1960 you wouldn't be sitting there thinking "ha, 'and'. Bet she's a red herring protagonist. Rookie mistake." The billing is probably an actor negotiation thing out of his control anyway.

You mean he put Miles in the shower too and filmed a fake shot of her?
I guess since I knew how it went I just assumed it was the former-but maybe not in 1960 if seeing it in a theater.
