Besides Hitler, Stalin, Mussolini, I'd add Idi Amin, Kim Jong-un, and Donald Trump to the list (the latter, not just because of what he has done thus, far but also because of what he is likely to do before he leaves office.)

Blood wise - one not mentioned was Genghis Khan and his descendants. GK alone might have been responsible for the deaths of 10-15 millions. For the populations of those conquered regions, that's brutal.
Incompetence-wise - One of the WWI vintage monarchs, William II or Nicholas II. If you went back to June of 1914 and say that the German and Russian monarchies would be swept away by 1918, never to return. you would have been scoffed at. Even Louis XVI didn't bollix things up so bad that no one would even seriously consider bring back the monarchy.
