Cinematic shock jocks like the Human Centipede or Saw are excluded.
For me, I was disturbed by both the Girl Next Door (2007) and the original Texas Chainsaw Massacre. I can't say why those films disturbed me whilst films like Cannibal Holocaust and Irréversible, whilst difficult to watch, did not leave any sort of lasting impression on me. Perhaps I was too young to have truly comprehended the gravity of them.
Regardless, which film disturbed you the most? What about it was so effective that it left you with a strong, visceral sense of disgust and/or horror?

Is Girl Next Door the movie where a mother and her kids keep a neighbor girl in their basement and do bad things to her? If so, then yeah, I'd agree. That movie is disturbing. I didn't make it to the end.
The Maniac remake is also pretty disturbing, as was A Clockwork Orange.
Overall, I avoid highly disturbing movies. For me, the concept of disturbing is more effective when used as a disturbing scene or two in a movie that isn't otherwise disturbing. When everything is disturbing, it feels too easy. Cheap.

Yes, that's the one. What makes it worse is the fact that it was based on the very real torture and eventual murder of Sylvia Likens.

Island of Death by Nico Mastorakis is the only movie without real violence that I've had trouble watching. Very well made and very disturbing.

What Lies Beneath jangled my nerves. I was actually trembling in the theater's ladies room afterwards so badly that my best friend asked me if I was OK. It was very creepy at the time. I was very young when I saw it and was terrified by it. I remember we rented Stir of Echoes the week after too which was worse.

What Lies Beneath is a glossy, star-driven, NON-SCARY movie that's more a thriller than anything else. Once you knew the ghost was reaching out to the main character, the end was telegraphed.

The most disturbing movie I saw as a kid was What Ever Happened to Baby Jane. Bette Davis scared me shitless as loony Jane! I remember what a revelation it was to discover that she'd actually been a romantic leading lady!
