This was a real problem in early talkies, and was hilariously spoofed in SINGIN' IN THE RAIN.
Well, it seems that problem still exists, at least on TV. I'm currently watching reruns of LAW & ORDER on Ion TV, and the lip movements don't match the dialogue, even on the commercials! I hope it could be brought to the attention of the network bosses and that they'll remedy the situation.

GWTW has a long segment of out of sync dialog. Whe Scarlet confronts Ashley, with Rhett listening, is horribly off. You think someone could have fixed it after all these years.
Fiftenn years ago, I went to a screening of the never released film, THE COMMISSION, at the ArcLight at the Cinerama Dome in Hollywood. The film was about the Warren Commission's investigation into the JFK assassination. The writer director was presenting the film and many of the cast were present, including Marin Landau, Ed Asher, And Joe Don Baker. Lloyd Bochner and Stephen Collins were sitting right behind us.
The Director had ordered a new print with a yellow tinge (to make it look old) for the showing. The entire second reel, which took 30 minutes, was 10 seconds out of sync. It would have been laughable if it hand't have been so sad for all involved.

All I can say is that it is very enervating when it happens.
I got an explanation once by a self appointed expert, that the cause for out of sync sound and picture had to do with having too long cables, and compared it to thunder and lightning.

...I own a DVD copy of the Jane Russell film "Outlaw" and it is impossible to watch because of the sound/voice synchronization...
