The Cove for me too. It's far too upsetting to sit through again.
I don't think anything fictional will bother me. I've seen The Human Centipede and August Underground series and thought they were ridiculous rather than disturbing. I've yet to see Salo or Martyrs, i have them but haven't got round to watching them yet.

Hard Candy. A guy gets neutrered by a little girl in a exceuciatingly long sequence that tied my stomach in knots. Actually, she fools him into thinking she did it by freezing his parts and going through the motions. Still hard to watch the first time when you think she's actually doing it.

The Hills's Have Eyes remakes and The Human Centipede. I have no desire to revisit those images.
As far as most disturbing emotionally, for me It's The Boy in Striped Pajamas, but It's one I recommend everyone watch. I really struggle with Holocaust movies anyway, but that one messed me up for days. It's very powerful.
I will also never again put myself through the emotional roller coaster that was Marley and Me.

I couldn't see The Exorcist and The Prince of Tides for the longest time. But then I did.
I still cant watch a little horror flick called Martyrs though. That one's tough, not for the scares, but for the sheer brutality from beginning to end.

That is a hard question to answer. I never get grossed out/disturbed by a movie enough to be discouraged to ever see it again, but one that has stayed with me is The Butterfly Effect (2004). I saw that years ago and haven't even sat through the whole thing, but in all honesty I cannot quite revisit it due to some of the insane things that happen. Particularly the sudden death of a mother and her baby. Just traumatizing in every way. Change this to Hereditary. Disturbing but for the right reasons.

Clockwork Orange Funny Games I have FGs on my list. Will add CO to it as well. I find it to be a big heap of trash and way too overrated.

It takes a lot to turn my stomach. One good film has done it, and one terrible film has. And a part of a decent film takes the cake for the most revolting thing I'd be happy never to see again.
-Good film that did it: "Irreversible" Quite liked this movie, but parts of it (those have seen it know the parts I mean) were unbelievably hard to watch. Some of the 'payoffs' revenge buffs might enjoy are...paid off, but it really is just piling on the pain. Intentionally disturbing, I'm sure. "Irreversible" is pretty raw and disturbing, but you gotta take your hat off, it's a solidly made movie. I'd recommend anyone with a strong stomach to check it out. Not on a first date though.
-Bad film that did it: "Seed" If you've never heard of this movie, don't bother. I can watch Rob Bottin's effects on "The Thing" in slow-mo, eating a bowl of cold spaghetti, and not miss a beat. "Seed" has some of the most senselessly vile and disgusting crap in any movie I've seen. If disturbing is your thing (I'm old enough to remember 'Faces of Death' and 'rotten.com'), this is the worst kind. Bleak, un-gussied up, and mean. The hammer scene was shockingly tasteless. Made "Cannibal Holocaust" look like "Blues Clues."
Decent film that did it the worst: "The Fly 2" Of course a big downgrade from the original (actually the second remake) with Jeff Goldblum. Decent enough creature feature with some good practical gore effects. Call me a dog man (I've been called worse), but the most horrible thing I've ever seen on film has to do with a certain golden retriever in this movie. I'm sensitive with dog stuff about on the same level as people stuff (let's talk about how horrible it must have been to be the toxic waste guy in 'Robocop'), but this is some sheer bastardry of man.

Clockwork Orange is too satirical to be disturbing to my mind, but as an overall film, I have never really bought into whatever genius Kubrick fans see in it. I would rather watch Nicholas Roeg's The Man Who Fell To Earth, which is poetic and elegiac and more subtly intelligent. I can sense Kubrick thinking in many of his films, due to his controlling nature. Yeah, that's a good one as bizarre as it was. Check out Brother From Another Planet if you haven't already.
